Description:

We are looking to expand our medication management team with remote, contract, clinical telepharmacists who share our passion for getting patient medication regimens right to drive better clinical and financial outcomes. Responsibilities:

  • Perform VBC comprehensive medication management, chronic disease state management, and medication reconciliation via virtual patient visits
  • Identify patients who have not achieved clinical goals of therapy
  • Identify how patients are taking all of their medications, including OTCs, herbals, DME and provide solutions for adherence/administration, inappropriate therapy, etc 
  • Evaluate each medication for appropriateness, effectiveness, interactions, and safety
  • Identify and manage drug therapy interventions 
  • Develop a care plan for the patient, provider, and/or payer
  • Consult with patients’ physicians to ensure clinical recommendations are considered
  • Conduct follow-up evaluations with patients and participate in other clinical activities as assigned by the Director of Pharmacy

Requirements:

  • PGY1 Pharmacy Residency Experience with at least 2 years of direct patient care experience in comprehensive medication management or ambulatory care (excluding clinical rotations in pharmacy school) 
  • You speak english plus Spanish fluently
  • You are a graduate of an ACPE-accredited school of pharmacy in the United States with a Doctor of Pharmacy degree. 
  • You possess a current, unrestricted pharmacist license and in good standing.
  • You have a valid NPI number
  • You possess and can show proof of malpractice insurance outside of your employer
  • You are actively seeing patients in your current or most recent role
  • You are familiar with federal and state pharmacy laws
  • You enjoy problem solving and always keep in mind what is best for the patient
  • You keep a positive attitude and genuinely enjoy talking to patients
  • You are able to be focused and productive while working from home
  • Encrypted computer with video required (Windows 10 Pro with BitLocker, Mac OS with FileVault, or Chromebook)
  • You have a steady and reliable internet connection
  • Minimum 24 hours of availability per week between the hours 1P-9P EST
  • Able to start working within 3 weeks of hire

Bonus points if:

  • You have completed an MTM training program by APhA or PowerPak C.E. 
  • You are board certified via BPS or NBMTM.
  • You have experience launching new patient care programs.
  • Available on weekends or after hours.
